Thank you for choosing our practice. Please e-mail or call us to schedule your appointment. Please mouse- over the “Before You Visit” link above for the sheets you will need to complete prior to your visit; Please complete and bring them in with you. Other items you will need for your appointment are: All current insurance cards and the referral number from your primary care physician (if you are a member of an HMO or POS plan).

If you have a co-payment, or have not yet met your deductible, please be prepared to pay this at the time of service. If your referring physician has performed x-rays and/or other tests or studies, please bring the films and test results with you. Also, please bring a list of all medications that you are currently taking (including dosage and instructions). If you are being seen due to an accident at work, or an auto accident, complete insurance information and authorization for treatment from the insurance carrier must be submitted to our office prior to your appointment.
