company for verification.
Referrals
If you participate in an HMO, Medicaid's Georgia Better Heath
Care or any plan that requires a referral to a specialist,
it is your responsibility to see that we have a referral authorization
from your primary care physician for each visit.
Insurance
Please make sure you provide us with a correct and current
copy of your insurance card at your visits. We file insurance
strictly as a courtesy to our patients. We ask that all co-pays,
deductibles, and other percentages not covered be paid at the
time of service.
Payment
We accept cash, checks or VISA and Mastercard.
Special Needs
It may be necessary to set up a payment plan under certain
circumstances. Special needs are understood by
this office. Please contact our billing department as soon as
possible if this applies to you. |
